Monthly Membership & Attendance Report
Your Monthly Membership and Attendance Report must be Filed with District 5110 no later than the 15th day of the month by submitting the online form below. Reports are summarized by 25th of the month by Attendance Secretary.
IMPORTANT NOTICE: Clubs must update their membership information by the last day of the month with Rotary International by using the Member Access Portal on the Rotary International Web Site. The RI Member Access Portal requires that all club Secretaries and Presidents to register in order to receive a User ID and Password to update all Club membership information and Club Data. Go to the link below to sign in or register in order to update your club membership information. Completing the form below does not update the Information with R.I.

NOTE: Please DO NOT include Honorary Members in your monthly report. Only include "Active" members.
NOTE: When you click "submit", you will NOT have the opportunity to review your input and make any necessary corrections as well as print this report for your records. Within one week you will receive an email acknowledgement from District Attendance Secretary that your report was received.
IMPORTANT - PLEASE READ
Report new members, terminated members, and member change information to Rotary International should be done ONLY ONLINE by using the "Rotary Member Access Portal" on the Rotary International Web site.
PLEASE NOTE: Club data at the Member Access Portal is only available to the current District Governor and Governor-Elect, and all Current Club Presidents and Secretaries and Executive Secretaries (per the 2008-2009 Rotary Official Directory).
